An exciting opportunity has become available to work within our growing Discharge to Assess service which facilitates the discharge of individuals from psychiatric hospital. It offers time-limited, specialist mental health support as individuals transfer from acute settings to virtual wards as a mechanism to improve the patient journey during transfer of care. The role of a Discharge Liaison Worker is to offer individuals support wherever they call home. We recognize that when a person may be experiencing a time of difficulty it is important to offer flexibility, dedication and compassion; therefore, these are the traits that we look for in members of our team. We work with a person-centered and recovery approach; empowering individuals to become as independent as possible. This service will provide a key function in facilitating a safe and timely discharge for patients within an acute setting. Our work will be solution-focused and be a core part of the multi-disciplinary ward meetings.
You will work in a varied and dynamic working environment with competitive rates of pay (£19,000 per annum pro rata) 40 hours per week and benefits scheme. You will receive full induction training, continued professional development and flexible working with including evenings and weekends. Excellent support from experienced management and the wider clinical leadership team plus an opportunity to get involved in this new and exciting service from the ground up.
Qualifications and Skills
You should have an enthusiasm and interest in working within the mental health sector with excellent verbal and written communication skills and a willingness to go the extra mile. You should have a flexible approach that respects individual differences with an ability to work as part of a team as well as independently, using your initiative.
